FRIENDS & FAMILY TEST

We have two practice nurses, who are very well qualified and able to provide chronic diseases management of chronic obstructive pulmonary disease (COPD), asthma, diabetes and high blood pressure. They can perform ECGs (heart tracings), removal of sutures and ear syringing. They are also available for wound care management, and provide family planning, travel and child health services. They can also help you to loose weight with dietry advice.
Is carried out by Dr Garsin on alternate Wednesday mornings and referred as necessary by your own GP.
Invitations for these are sent out by the PCT to female patients between the age of 25 to 64 years, patients are asked to make an appointment with one of the practice nurses.
Advice and vaccinations can be provided at the clinic on Wednesday afternoons. Patients are requested to complete a travel questionnaire prior to attending the clinic to help clarigy their requirements and it is strongly recommended that sufficient time is allowed prior to travelling to enable full courses of vaccinations to be completed. Some vaccinations are available on the NHS but others may incur a fee. A list of these fees is available at the reception desk.

Every September we start our flu immunisations and contact all our patients on the ‘at risk’ registers patients who have (Asthma, Chronic obstructive pulmonary disease (COPD), Diabetes, take Immunosupressant medication, Over 65 years of age, Ischaemic Heart Disease (IHD), Kidney disease, had a Stroke, Neurological disease or if you are a carer to come in and be vaccinated. Please get in touch with us if you are not contacted to come in if you think you are entitled to a flu injection. Other patients who do not fall within these groups and wish to be vaccinated can approach their local chemist to have the vaccination.
Our doctors and nurses are able to advise you on the most appropriate form of contraception. We can provide free condoms and a prescription will be given for ‘the pill’. As well we can perform coil and implant insertions by appointment only for 20 minutes and requires both the Nurse and the Doctor.

There are times when we need to contact you so it is important that you keep us up to date with your personal details such as change of address or telephone number
Our attached District Nursing team are skilled at providing nursing care and advice to people of all ages in the community. They can be contacted on 01895 234001 by leaving a message.
Health Visitors are based at Uxbridge Health Centre they can be contacted on 01895 488850.
Counsellors are in regular attendance at the surgery a referral can be made by your GP or you can self refer (see information on self referral tab)
Extended Access Hubs
There are now appointments with GPs and nurses available to book outside of the standard GP practice opening hours. Ask at reception desks for availability.

Care Connection Team
Care Connection Teams (CCTs) look after patients aged 65 years and over, who have been identified by the GPs as needing case management because of their health and social care needs.
